These forms must be completed by a licensed
health provider and returned to the Student
Health and Wellness Center no later than four
weeks before you enroll in SHRP.
Note: If the University’s director of
Student Health finds the results of this medical
evaluation to be unacceptable, the School reserves
the right to revoke the acceptance. In that
case, the tuition deposit will be refunded.
Keep copies of your completed medical forms.
You will be required to present verification
of some vaccinations/tests at affiliated clinical
sites prior to each of your clinical rotations.
